Supposedly for isolating and supporting the testicles, I still don't understand why these exist besides sexual purposes.
There's just something about them that I can't wrap my head around. I mean, support can be found in other forms of underwear. And while I'm sure there's a circumstance, I'm not sure when isolation would be needed. And if you're just going to put a pair of pants or shorts on top of these, are you really isolating them from anything?
I'd prefer the jet brief or the jet thong, if they do the job. Or, maybe even this leather pouch! (Warning, penis pic.)

I had prostate cancer surgery. I woke up wearing a suspensory and a catheter. It was a strange feeling but my balls needed support - very swollen and painful - and I had this drain line running out of me. Practical.

If you really need to have your testicles held snugly at all times then you need one of these. Briefs or a jockstrap will hold you nice and tight too, but you need to pull them down to use the bathroom and that can be painful. And try having sex with briefs or a jockstrap on!
